* crewai eval --models: per-run cost and time, and no repeated progress lines

The comparison table printed each setup's cost and time summed over all
its runs, while the brief and the report say them per run. It now prints
cost_per_run / seconds_per_run under 'cost / run' and 'time / run' (the
totals, headed 'cost' / 'time', only when the service sends no per-run
figure; a column never mixes the two).

Progress deduped only against the previous line, so parallel setups made
polls alternate and the same lines printed again and again. Each line is
now said once per wait.
