Greyson LaLonde 63fc2e7588 fix: complete recursive MCP schema handling ...

resolve_refs now returns type-preserving stubs instead of {} for
circular $refs, and create_model_from_schema catches JsonRefError
to fall back to lazy top-level-only inlining.
